Here is how to get your lost passport replaced,

  1. Contact the Ghanaian Embassy in your home country for a copy of the application form. Fill out an application form with complete information. Submit a letter explaining that you want to renew your passport, pay a cost of $30 to cover the cost of the form. You can also download the form from their website, but you still have to pay the fee when you submit your application.
  2. Pay to cashiers check for $100, the processing fee, made payable to the Embassy of Ghana, if you are mailing in your application. If you deliver the application in person, you can pay in cash.
  3. Provide One Passport Photo.Do not bend the photo.To avoid processing delays, be sure your photo meets all the requirements and is properly submitted with your application. Enclose four color passport-size photos if you sending by mail, which have been taken within three months of the date you apply for your passport renewal. The photos must be taken with a white background and show your full face; dont wear a hat or dark glasses. One of the pictures must be signed by your witness as a true likeness of you.
  4. Return the completed application form, old passport, photos and money order to the Embassy, along with a pre-paid FEDEX, UPS or USPS envelope. The Embassy stresses that you must use a tracked envelope instead of a regular envelope due to the risk of it going missing in transit. For the same reason, you should also use a tracked courier service when sending your documents to the Embassy.
